What stays off the plate

The Enemies

Sugar, ultra-processed food, and synthetic additives: the evidence against each, the gray zone around flour, oil, and alcohol, and how to read a label once and be done.

The Earth Diet is defined as much by what it leaves out as by what it puts on the plate. Three things do most of the damage, and all three are avoidable without a prescription or a gym membership.

01Sugar, the saboteur

Glycation · insulin · the dessert counter

Excess sugar ages you through a process called glycation: sugar molecules bond to proteins and form advanced glycation end products, aptly shortened to AGEs. Collagen and elastin, the proteins that keep skin springy and arteries supple, are long-lived and therefore accumulate this damage for decades. Glycated collagen is stiff, brittle, and yellowed; the fine lines you fight with serums are partly built at the dessert counter. MECHANISM

The population evidence is blunt. In a national U.S. cohort followed for about 15 years, adults getting a quarter or more of their calories from added sugar had nearly three times the cardiovascular mortality of those getting less than a tenth. COHORT Sugar-sweetened drinks are the single largest source, and they track with type 2 diabetes, weight gain, and fatty liver in cohort after cohort. COHORT The American Heart Association's ceiling is about 36 grams of added sugar a day for men and 25 for women; one soda spends the entire budget. CONSENSUS

What sugar is not

Whole fruit is not the enemy. Its sugar arrives wrapped in fiber, water, and polyphenols, eaten slowly, and fruit intake tracks with lower, not higher, mortality. COHORT Juice is a different matter: fiber removed, sugar concentrated, swallowed in seconds. Eat the orange; skip the glass. Raw honey is the Earth Diet's one deliberate sweetener, measured in threads rather than spoons, because it carries trace polyphenols and because a thread is enough once the palate recalibrates.

The swaps

02Ultra-processed food

NOVA · the Hall trial · ten million people

Processing is not the problem; humans have milled, fermented, dried, and canned for millennia, and plain yogurt, canned beans, frozen berries, and olive oil are all processed foods that belong on the plate. The problem is a specific category. In the NOVA classification used by researchers worldwide, ultra-processed foods are industrial formulations made mostly from substances extracted or derived from foods (modified starches, protein isolates, hydrogenated oils, high-fructose syrups) plus additives that make the result look, taste, and feel like food: emulsifiers, colors, flavors, sweeteners, and texturizers. Packaged snacks, soft drinks, instant noodles, reconstituted meat products, most breakfast cereals and bars, and most fast food qualify. CONSENSUS

The decisive experiment was run at the U.S. National Institutes of Health in 2019. Twenty adults lived on a metabolic ward for a month and were fed, in random order, two weeks of an ultra-processed menu and two weeks of an unprocessed menu matched for calories, sugar, fat, fiber, and macronutrients, with permission to eat as much or as little as they liked. On the ultra-processed menu they ate about 500 more calories a day, ate faster, and gained roughly two pounds; on the unprocessed menu they lost the same. RCT

At population scale, a 2024 umbrella review in The BMJ pooled 45 meta-analyses covering almost ten million people and found higher ultra-processed food intake consistently associated with higher risks of cardiovascular death, type 2 diabetes, common mental disorders, obesity, and death from any cause. COHORT Association is not proof, but the trial above supplies the mechanism, and no study has ever found the opposite.

Why it works against you

The test

Could you picture it growing?

Look at the ingredient list. If you can picture every item on a farm, in an orchard, in the sea, or on a spice rack, it is food. If the list reads like a chemistry set, or you cannot imagine making it in a home kitchen, it is a product. The Earth Diet eats food.

03Synthetic additives

Emulsifiers · sweeteners · dyes · nitrites

Additives are approved one at a time, mostly for short-term safety, by agencies that do not test them in the combinations people actually eat for decades. The record on several is turning, and the Earth Diet applies the precautionary principle: if it did not grow anywhere and the evidence is unsettled, leave it out.

Turning

Emulsifiers

Polysorbates, carboxymethylcellulose, carrageenan, mono- and diglycerides. In animal studies they thin the mucus layer that keeps gut bacteria off the intestinal wall and drive inflammation; in the French NutriNet-Santé cohort of about 95,000 adults, higher emulsifier intake tracked with higher cardiovascular disease and cancer risk. MECHANISMCOHORT

Turning

Artificial sweeteners

In 2023 the World Health Organization advised against non-sugar sweeteners for weight control, citing no long-term benefit and possible harm; the same year the International Agency for Research on Cancer classed aspartame as possibly carcinogenic (Group 2B). Cohorts link high intake to cardiovascular disease. Sparkling water with citrus is the swap. CONSENSUSCOHORT

Settled

Nitrites and processed meat

The International Agency for Research on Cancer classifies processed meat (bacon, ham, hot dogs, deli meat) as a Group 1 carcinogen for colorectal cancer, on the strength of the human evidence; nitrite curing is one proposed mechanism. The Earth Diet has no place for it. CONSENSUS

Turning

Synthetic dyes

Red No. 3 lost its U.S. food authorization in January 2025 after decades of animal cancer data; several other synthetic dyes carry behavioral warnings in Europe. Titanium dioxide was banned as a food additive in the European Union in 2022. Color on the Earth Plate comes from anthocyanins and carotenoids, which are the point. CONSENSUS

Precautionary

Preservatives and phosphates

BHA and BHT, propyl gallate, and added phosphates in processed meats, colas, and baked goods. Evidence ranges from animal data to associations with kidney and vascular stress at high intakes. Whole foods do not need them. MECHANISM

Precautionary

Flavor systems

"Natural flavors" and "artificial flavors" are proprietary blends that can contain dozens of compounds. They exist to make a product crave-able; a lemon, a clove, and a sprig of rosemary do the same job with their chemistry intact.

The gray zone

Refined flour · refined oil · alcohol

Refined flour is not an additive and not poison; it is simply a whole grain with its fiber, germ, and most of its minerals removed, which is why white bread and pastries behave like sugar in the bloodstream. The Earth Diet replaces them with intact grains as defaults and treats good sourdough and whole-grain bread as the occasional exception.

Refined seed oils have become a lightning rod. The honest reading of the evidence is that replacing saturated fat with polyunsaturated oils lowers cardiovascular risk in trials, RCT and the alarm about omega-6 fats is not supported by human data. But most seed oil in the modern diet arrives inside ultra-processed food, often repeatedly heated, and extra-virgin olive oil has positive trial evidence that refined oils do not. So the Earth Diet uses olive oil as the default, not because seed oils are toxic but because olive oil is better and the company it keeps is better.

Alcohol was part of every Mediterranean cohort as wine with meals, and that is the only context in which the Earth Diet accommodates it. The World Health Organization's 2023 position is that no level of consumption has been shown to be safe; alcohol is a Group 1 carcinogen; and it fragments the second half of the night's sleep. CONSENSUS If you drink, keep it small, with food, and never within three hours of bed. If you do not drink, nothing here asks you to start.

The three-week reset

Give it three weeks. Taste recalibrates fast: within a month of leaving added sugar and ultra-processed food behind, fruit starts tasting like candy again, plain yogurt stops tasting sour, and the old products taste the way they are, which is too sweet and too salty at once. That is the earth's original arrangement reasserting itself, and it is the point at which the Earth Diet stops being an effort and becomes a preference. The 20-week stack schedules the first sugar swap for week eleven for exactly this reason: by then the bowl, the berries, and the fish have made the swap easy.
